The Mental Health Professional provides comprehensive clinical services to adults with mental health diagnoses. This position manages the development and implementation of individualized service plans (goal plan, behavioral plan, etc.) and ensures comprehensive care is being provided.

Additional responsibilities include file reviews, data collection, reporting and facilitating clinical training based on the needs of program staff.

This position will meet with individuals out in the community or in the individual’s home in one on one or group settings of 5 or less. They will provide support and guidance to individuals in building and developing skills related to health and wellness, life skills, education, working, and socialization. This position will utilize an electronic health records system to complete progress notes for each service rendered that aligns with the individual’s goals.
